FOOTBALL/CHEERLEADING
2011 SEASON

A copy of a birth certificate is required for all new sign ups

FOOTBALL:
Must turn 8 yrs old by Dec 31st and not going into the 9th grade

**** 8th grade students can play with either PFY team or on the school team but not both ****

Registration Fee: $85.00 (includes clinic)
Jersey Fee: $60.00 (those who need jersey, this includes two jerseys)
(There will NOT be a fundraiser this year)

CHEERLEADING:
Must turn 8 yrs old by Dec 31st and not going into the 9th grade.

Registration Fee: $50.00
Warm Up Suit: $45.00 (if needed, all new girls will need a warm up suit)
(There will NOT be a fundraiser this year)

There will be a $25.00 late sign up fee charged after this date June 10, 2011

*There will be no mail in registrations*

All sign ups received only require a $25 deposit, the balance is due by August 10.

Last day to sign up is August 10th for both football and cheerleading.

*Football/Cheerleading Commissioner has the authority to accept or refuse sign ups after August 10th
